1. All patients MUST complete this online assessment prior to your appointments:


You do not need to send us a copy but if you answer yes to any of the screening questions, please inform us immediately and treatment may be delayed.


2. The appointments will be staggered to limit direct contact with other patients.  Please arrive on time for your appointments so we can maintain appropriate flow.  Arriving too soon or too late will create an undesirable overlap.  There may be greater wait times given new protocols, so please allow adequate time for your appointment in your personal schedules. 


3. Please come to your appointment alone and on time. Children can be accompanied by only one parent.


4.  Payment options will be limited to debit or credit cards, processed at the time of your appointment.   

Credit card payments will be processed by our team after your appointment. 

We will continue to process insurance claims, however payment will be due at the time of treatment to minimize handling of paperwork and direct interaction. 


5. Upon entering the office, you will have your temperature taken, be asked to wash your hands with soap and water, and to rinse your mouth with a disinfectant rinse.  


6.  Waiting rooms are closed, and we will guide patients to minimize crossover in the office. 


7. Wherever possible, we ask that you wear a mask or face-cover to protect yourself and those around you.
